When ODSP grants disability status, the decision letter often specifies whether a Medical Review is required in the future (typically 2 to 5 years after approval).

What to Expect in the Medical Review Package
When your medical review date arrives, the Disability Adjudication Unit will send you a Medical Review Package:
- Medical Review Form: Needs to be completed by your physician, nurse practitioner, or specialist.
- Self-Report Form: Gives you the opportunity to describe how your condition continues to restrict daily activities.
- Submission Window: You are given 90 days from the date of the letter to complete and return the package.
