Understanding ODSP Medical Reviews

When ODSP grants disability status, the decision letter often specifies whether a Medical Review is required in the future (typically 2 to 5 years after approval).

What to Expect in the Medical Review Package

When your medical review date arrives, the Disability Adjudication Unit will send you a Medical Review Package:

  • Medical Review Form: Needs to be completed by your physician, nurse practitioner, or specialist.
  • Self-Report Form: Gives you the opportunity to describe how your condition continues to restrict daily activities.
  • Submission Window: You are given 90 days from the date of the letter to complete and return the package.

Book an appointment with your doctor as soon as you receive the package. Do not wait until the last week, as medical clinics often require several weeks to complete paperwork.
