The 2-Step ODSP Application Process

Step 1

Financial Eligibility Assessment

Your local ODSP or Ontario Works office reviews your income, liquid assets ($40,000 limit for single individuals), housing situation, and family composition to confirm financial eligibility.

Step 2

Disability Determination Package (DDP)

You will receive a medical package containing the Health Status Report and Activities of Daily Living forms to be completed by your doctor or healthcare provider.

Completed by your doctor, physician, or nurse practitioner detailing your medical conditions and treatments.

Activities of Daily Living

Documents how your impairment impacts self-care, mobility, work capability, and community living.

Self-Report Form

Optional but highly recommended form where you describe in your own words how your disability impacts daily life.

Appealing a Denied ODSP Application

If your application or medical eligibility is denied by the Disability Adjudication Unit (DAU), do not lose hope. Over 50% of appealed denials are overturned in favor of applicants.

  • Stage 1: Requesting an Internal Review (Within 30 Days)

    You must write a letter or complete an Internal Review Request form within 30 days of receiving your denial letter. State clearly that you disagree with the decision and request an internal review. You can attach updated medical evidence.

  • Stage 2: Social Benefits Tribunal (SBT) Appeal

    If the Internal Review upholds the denial, you have 30 days to file an Appeal Form with the independent Social Benefits Tribunal (SBT). An independent tribunal member will hear your case.
