Understanding DAU Disability Medical Reviews

When ODSP grants disability status, the Disability Adjudication Unit (DAU) frequently sets a scheduled Medical Review date (typically 2 to 5 years down the road) to check if your medical condition has improved.

Receiving a Medical Review package in the mail can feel stressful, but following proper preparation ensures your benefits continue smoothly without interruption.

Key Components of the Medical Review Package

When your review arrives, the package contains two main parts:

  1. Medical Review Form (Parts A & B): Must be filled out and signed by your physician, specialist, or nurse practitioner.
  2. Self-Report Form: Gives you the opportunity to describe in your own words how your medical conditions restrict daily personal care, mobility, and work capabilities.

Essential Tips for Success

  • Respect the 90-Day Deadline: You have exactly 90 days from the date of the letter to return the package. Book an appointment with your doctor immediately.
  • Requesting Extensions: If your specialist appointment is delayed beyond 90 days, write to the DAU before the deadline requesting a formal extension and include proof of your upcoming appointment date.
  • Detailed Physician Statements: Remind your doctor to detail your functional limitations (e.g., standing, sitting, lifting, fatigue, cognitive restrictions), not just diagnostic labels.

Do not minimize your struggles on the Self-Report Form. Describe your worst days, the assistance you require, and how your disability impacts your daily life.
